Concierge Direct Primary Care (DPC) - A Personalized Approach to Your Health & Wellness

DPC is a membership model with a monthly flat rate and enhanced family medical care.

We don't bill your insurance or charge for individual visits and provide different services from regular insurance visits. We offer straight-forward affordable care and a relationship over time.

Easy access to Dr. Lillig via email, text, or phone call with quick and personalized responses! Visits are in-person at BSIH or virtual telehealth --- whichever is most convenient!

Avoid lengthy, in-office waiting and being treated like a number!

Keep your traditional insurance for high-priced emergencies (hospital/surgery), specialists, and pharmacies. DPC provides real access to everyday health needs.

No per-visit charges or charges for co-pays, point-of-care testing, or contacting doctor
Routine bloodwork through major carriers like Quest and Vibrant Health
No "surprise fees" or complicated billing and insurance paperwork
Use HSA funds for membership fees, no problem!
Examples of care: physicals, diabetes, hypertension, orthopedic injuries, weight loss therapy, back and joint pain, mens health, sick visits (sinus, ear, rash), anxiety, depression, ADHD, insomnia, geriatrics, nutritional counseling, hormonal replacement therapy, palliative care, dermatology, and holistic health approaches.
Routine bloodwork includes: CBC (blood counts), CMP (liver, kidney and electrolytes), A1c (diabetes check), FLP (cholesterol), Thyroid Panel, Urinalysis. May also add Women or Mens Hormone Panel (estradiol, testosterone, progesterone, etc) PSA (prostate),
Custom compound medications, medication supply, lab testing

Standard Enrollment

Tele-health and in-person visits are available now!

Enroll online and complete the entire form - we will then confirm your enrollment and send direct scheduling and clinic contact information to confirmed patients.

If we cannot process your enrollment within two weeks, we will send notification of enrollment dates and/or refund any paid fees. We reserve private access and contact information for the clinic/doctor/nurse to actively enrolled patients only.

Direct Primary Care Membership

One-Time Enrollment Fee: $100
(Required at signup for all new Direct Care members)

Customized primary care that’s personal, accessible, and tailored to you — with easy self-scheduling, email/text communication, and responses within the next day for most medical questions and concerns.

Our Direct Care model gives you direct access to your care team without traditional insurance barriers — ideal for patients who want personalized care, continuity, and convenience. Yes you can use HSA/FSA funds and also see other routine, insurance-based providers!

Membership Options (billed monthly):

Individual Plan — $55 / month
Enhanced Plan — $75 / month (Age >60)
Family Plan — $150 / month
Unlimited children included in family plan!

Counseling & Integration Therapy - $135/hr

Counseling & Integration Therapy provides supportive, integrative conversations that bridge medical insight, psychological understanding, and real-life application. Sessions are practical, compassionate, and non-judgmental, with an emphasis on turning insight into sustainable change.

Family Health Counseling addressing health, lifestyle, relationships, and life stressors
Motivational Interviewing & behavior/lifestyle/addiction support
Integration Counseling for processing meaningful life experiences or treatments
Stress, burnout, and life transition support

Ketamine-Assisted Therapy (KAP)

A therapeutic program combining psychiatric evaluation and guided ketamine sessions to support mental wellness. Special packages for first responders, military, health professionals.

What’s Included in Initial KAP Package:

One-hour preparatory consultation and exam
180-minute supervised dosing session (IM, SC, oral, or sublingual dosing)
15-minute integration counseling
One-week follow-up integration visit

Initial Session: $650 (includes screening), Repeat Sessions: $595 each
Package Option: Five sessions for $2,495

Adolescent Health Consult — $60 ($20/month)

Supportive, confidential care designed for teens and young adults (age 14–22).

Physical exams & sports physical
Mental health care and screening

STI screening and counseling
Supportive discussions for teens and families

Compound Pharmacy Consult — $100 / 6 months

For patients who need specialized or individualized prescriptions (e.g., bio-identical hormones, injectables, custom compounds).
We collaborate with your pharmacist or specialist and support maintenance medications with a semi-annual consult (telehealth or in-person).

To establish care: enroll with BSIH and a care team member will follow up to review your chart.

Medication Reduction Visit — $330

A 90-minute consult with a Board-Certified MD focused on safe deprescribing and optimizing medication regimens. Ideal if you have:

Multiple chronic conditions or specialists
More than five prescription medications
Complex medication interactions

Includes:

Full review of all medications & supplements
Coordination with specialists and pharmacists
Personalized taper plan and medication list
Family participation encouraged
